If you are considered Church Personnel as defined by the "Sexual Misconduct Policy" of July, 2003, you will need to complete the following steps:

 

Step 1 - Read the Diocese of Saint Cloud Sexual Misconduct Policy.
Step 2 - Print out and sign the Acknowledgment of Receipt form and give it to your program director.
Step 3 - Print out and sign the Background Questionnaire form and give it to your program director.
Step 4 - Read the Ethics and Integrity Guidelines.
Step 5 - Print out and sign the Acknowledgment of Receipt form and give it to your program director.
Step 6 -
 

 

Volunteers (Unpaid Church Personnel) need to printout, and sign with notarization, the "Mind Your Own Business, Inc." background check release form.

Paid Church Personnel need to printout, and sign with notarization, the State of Minnesota-Bureau of Criminal Apprehension background check release form.
